Initially from the Azores, the doctor was always fascinated by plastic surgery, that is, his aesthetic side. For six years, Rui Lima studied at Loco in the various areas of plastic surgery. The doctor went to Canada and the United States of America to study more about cosmetic surgery and has been working on the up clinic since 2018.

Portugal News (TPN): What exactly is blepharoplasty?
Rui lima (rl): Blepharoplasty is an eyelid surgery. When we evaluate a patient’s face, we should never forget the regional area, because the region region transmits many emotions. A more swollen folding area gives a more tired appearance and that, once again, in interpersonal relationships, it can have significant implications.
Therefore, regional rejuvenation is fundamental and although it seems like a small area, it is of great importance in terms of rejuvenation. When we do a regional evaluation, we have to think of the upper eyelid, the lower eyelid and never forget the eyebrow. Because these three ingredients should be treated together. Therefore, upper blepharoplasty is the surgery involved in the upper eyelid, the lower blepharoplasty is the surgery involved in the lower eyelid and the lift is the surgery that is involved in the eyebrow.
TPN: Can you explain the three types of surgery?
RL: In the upper blepharoplasty, the most common procedure is the treatment of excess skin. It is the most common surgery, it is the accumulation of the skin that gives an older look to the upper eyelid and we can remove this skin, leaving the scars well hidden and sometimes practically subtle.
Combined with this removal of the skin, we can also remove excess fat from the eyelid, which is not mandatory. We need to evaluate on a case -by -case basis, but it is very common to reduce the amount of fat. In some cases, in the upper eyelid, we sometimes have to raise the eyelid, which is called a blink of eyelids, and then we have to have a muscle surgery in the muscle to achieve a more effective eye opening.
In terms of the lower eyelid, the fat the treatment ingredient is very important, because this is often the case with aging and a less attractive appearance in the upper area, in the lower eyelid. What we do is reduce and reposition of fat bags. We do this through a technique called Canthopexy, basically in the corner of the lower eyelid, where we have a joint structure. We make a compression that brings a newer look to the lower eyelid. Combined with all this, we cannot forget the eyebrow, where we often believe that there is a surplus of the skin on the upper eyelid, but the problem comes from above. It comes from the falling eyebrow.
UP clinic is the only clinic that performs endoscopic lift surgery. What we do is an incision in the scalp so that we do not have scars, and through this cut in the scalp, we use cameras, with endoscopy, releasing the eyebrow joints and climbing without visible scars.
TPN: Apart from aesthetic reasons, are there any reasons for physical health that suggest the need to become this type of process?
RL: In fact, we have many surgeries performed by the ophthalmologist’s review, that is, the functional commitment to the eye. Here, the most common reasons are superior blepharoplasty, that is, a very intense excess skin from the upper eyelid, in which this excess is so large that it limits the opening of the eye. Therefore, people may have been affected by excess.
Or when, and this is due to damage or congenital deformities, in children already born with this congenital problem, in which the eyelid falls and then we must make the eyelid lift. From the lower eyelid, the most common is really the bump, when the eyelid already falls with amputation.
TPN: What are the procedures before and after surgery?
RL: In terms of pre-surgery preparation, we always make a checklist in terms of overall health of the patient. We must always do a detailed evaluation to check if hemoglobin is good, if the kidney function is good, just to see if, from the general view of body homeostasis, everything is well to support surgery and anesthesia. Because when we do surgery in which the eyebrow is combined with the upper and lower eyelid, it must be with general anesthesia.
Simplier surgeries can be performed under local anesthesia, such as isolated upper eyelid surgery. In addition, we always do heart tests, electrocardiogram and also radiography to make sure the patient is okay to do this surgery.
Usually, surgery recovery is simple. Obviously, there may be some discomfort in the first 3 to 5 days, which may include some pain that always needs analgesic support. The eyes and eyelids will be more swollen. On the third or fourth week, swelling is already less noticeable and than Tuesday or fourth week, you can already have a normal social life. The end result can be observed 1 to 3 months after surgery. Anyone who goes through surgery should not work for at least two weeks and stop physical activity, which can be repeated four weeks after surgery.
It is necessary to sleep on your back for the first two or three weeks, take the analgesic drug we give, a few drops to the eye, so that the eye is always very well hydrated and virtually, in the first two weeks, a small ice in the peripheral area to reduce swelling.
In the UP clinic, we have a hyperbaric chamber, which is a machine in which there is a peroxyx of tissues. The patient will remain one hour in the chamber breathing with 100% oxygen. This causes greater oxygenation of the tissues and therefore the recovery will be faster.
TPN: Why should people do surgery on up clinic?
RL: Essentially, due to our differentiation in plastic facial surgery. As I said, for example, for the eyelid surgery, we believe it is such a small area and a simple area, but the opposite, it is a very differentiated area, in which anatomy is very complicated and is really important to become a specialist in the area, because it requires not only specific materials, but also a lot of specific materials.
TPN: Do you think demand for surgery is increasing?
RL: Yes, according to ISAP (International Aesthetic Plastic Surgery), surgery most performed more in plastic facial surgery last year, that is, increased in such a way that the number is a kind of surgery worldwide and this is a bit in accordance with what is happening.
We have seen this development in the clinic and, as I said, it is important to do a personalized evaluation and then to have a technically differentiated surgery, also with the best materials and with experienced surgeons. Keep in mind that when there is a higher blepharoplasty in an isolated way only, it can be done with local anesthesia, but normally, when it is a large surgery with a lower, superior and lifting of eyebrows, we must do so with general anesthesia.
