Simone Biles continues to be her transparent self.
The 28-year-old Olympic athlete opened up exclusively to PEOPLE about her decision to undergo cosmetic surgery and why it’s important for her to speak publicly about the issue now.
“I’m just being who I am, and I feel like I’ve always tried to be open and honest and relatable,” she tells PEOPLE exclusively, before diving deep into her procedures, why she had them, and why others shouldn’t be ashamed of their choices.

The conversation arose shortly after Biles’ revelation TikTok that she has undergone three plastic surgeries so far and urged her followers to guess the work she had done. However, he noted that two of the three would not be noticeable.

The seven-time Olympic gold medalist did not specifically mention the surgeries she underwent in the video, but did respond to several of her followers who speculated in the comments section at her request.
While Biles played coy about her breast augmentation online by simply adding the cherry emoji, she was transparent about her lower blepharoplasty (which reduces tightness from the lower lids and removes excess skin from the upper lids, per Cleveland Clinic) and earlobe surgery.
“Seeing me win medals at the Olympics, that’s not comparable,” Biles tells PEOPLE exclusively. “But what is relevant is how we feel about ourselves, what we talk about, what we go through and how we share openly, honestly.”
“And that’s why I decided to share it,” she says of her social media post.
During the interview, Biles doubled down on TikTok that two of her surgeries are “things you’d never notice” and explained why she had to do them.

“I got lower mucus because me and my family have, I call them ‘Biles eye bags,'” she explains. “We just always had them, inherited, and so that was just a big flaw. Every time I went on a shoot, they were like, ‘Oh, can we put some stripes on her?'”
“And I said, ‘Oh, baby, I had 12 hours last night, these won’t go away. Those are just hereditary,” she shares, adding, “Well, it’s something I wanted to fix.”
Biles then explained that she had “plastic surgery” on her ear “to fix it” because one of her earrings “torn off” when she was a child.
Afterwards, Biles opened up about her breast augmentation, but emphasized that it wasn’t because she “didn’t like” the way she looked before getting it.
“Okay, obviously you can notice the chest,” she says. “But other than that, it’s just about feeling good about yourself and loving yourself, and I’ve always been very vocal about that.”
“And it’s not that I didn’t like how I looked or how I felt,” she clarifies, explaining, “it’s just something that I obviously notice a little more because I live in this body, I’m just used to it.”

Biles then shared the message she hopes the younger generation will take away from their transparency on the topic.
“I also think it shows young girls that they have a right to their own choices, whatever they are, and that’s not a shame,” she says. “I feel like in this day and age with social media, you look at everybody and you’re like, ‘Oh my god, how is she so good?’ ”
“Social media isn’t real, so I try to be as transparent as possible,” he continues. “But that’s why I also told them that on my 27th birthday I had botox – and it didn’t go well for me. I didn’t like it because I had a floating eyebrow and everyone was asking what that was.”
“So I tried to share that,” the athlete says of it in August 2024. “And it’s like, you don’t have to do these things to get in line to look anything.”
Biles concludes, “As long as you love yourself, that’s really all that matters.”
