Blepharoplasty or blepharoplasty is a life-changing procedure that can significantly affect one’s appearance and psychological well-being. For many, droopy eyelids not only distort their visual clarity, but also contribute to feelings of tiredness and aging. However, what was once primarily a medical procedure aimed at improving visual function has become an increasingly popular choice for cosmetic enhancements.
A 58-year-old man from Dubai opted for upper blepharoplasty due to persistent headaches caused by his drooping eyelid. The eyelid blocks his vision and contributes to a tired appearance. The procedure, which took about an hour, significantly improved his condition. Dr Rafic Antonios, an ophthalmic plastic surgeon at Moorfields Eye Hospital in Dubai, said: “Six months later, the man was cured and felt more confident both at work and in his social life.”
Dr. Antonios explained Khaleej Times: “Blepharoplasty addresses common concerns such as sagging skin, puffiness and tired eyes. These issues can make a person look older or more tired. Correcting them, surgery not only improves the appearance, but also restores a cleaner and more comfortable sight.”

Possible complications
While blepharoplasty is generally safe, patients should be aware of potential complications. Dr Alia Issa, cosmetic ophthalmic surgeon at Moorfields Eye Hospital, described some common post-operative issues as “complications including swelling and bruising around the eyes, temporary dryness or irritation and difficulty closing the eyes completely during healing”.
She further explained that swelling and bruising around the eyes are common after blepharoplasty and can cause initial discomfort. Patients may temporarily have difficulty closing their eyelids, leading to dryness or irritation. Although rare, complications such as infection, bleeding or vision changes can occur. Poor surgical technique or patchy healing can result in visible scarring or asymmetry, sometimes requiring revision surgery for optimal results.

Aesthetic improvements
The landscape of blepharoplasty has shifted from basic therapeutic procedures to selective aesthetic enhancements. Many are now pursuing cosmetic eye surgery to achieve trend-inspired looks such as the ‘fox eyes’ look. This increase in demand underscores the need for caution, the doctors stressed, noting that “healing from surgery can vary in complexity.” They added that individuals should weigh the potential benefits against the risks before proceeding.
The ‘fox eye’ look has been attributed to Bella Hadid although the famous model has denied having any surgery to achieve it, attributing her eye shape to make-up techniques and the use of facial tape.
Alarming case studies from a Dubai-based hospital highlight concerns about the risks associated with the procedure. A 32-year-old woman who underwent cosmetic surgery experienced serious complications, including corneal perforation, due to surgical errors. This patient refused the additional necessary surgery, which resulted in significant vision loss.
In another case, a 65-year-old man underwent blepharoplasty in which a previous hospital removed too much skin from the lower eyelid, resulting in an ectropion (folding of the eyelid outward). This condition causes chronic eye irritation. “The eyelids are vital to protecting the eyes and improper function can lead to significant problems,” said Dr Ania Buigues Llull, ophthalmologist at Barraquer Eye Hospital Dubai.
Dr. Anya emphasized the importance of recognizing post-operative warning signs. Scarring can occur due to improper healing, genetics, or specific medical conditions. He stressed the importance of skilled surgical technique, proper wound care and avoiding smoking.”

“Uneven eyelids can result from asymmetric tissue removal or healing differences. Accurate preoperative planning and experienced surgeons help minimize the risks,” he added.
Be realistic
Demographic factors play an important role in the success of blepharoplasty. Older patients may have slower healing rates and thinner skin, increasing the risks for complications. Skin tone can also affect healing, as darker skin may be more likely to have hyperpigmentation or keloid formation.
Dr. Ania emphasized the importance of realistic expectations. Dissatisfaction can arise even from technically successful surgeries due to unrealistic goals.
Precautions
Meanwhile, Dr Shahrzad Mojtabavi Naeini, an aesthetic GP at International Modern Hospital Dubai, said patients should take precautions before surgery. Here are some tips:
- Quit smoking weeks before surgery to aid healing
- Check health conditions—blood pressure and diabetes should be managed effectively
- Be careful with medications—certain medications such as warfarin, aspirin, and ibuprofen increase the risk of bleeding. These medications should be discontinued as advised by your healthcare provider.
