Nagpur: Although the third molar, commonly known as o wisdom toothis no longer required and is considered a residual dental part, doctors they feel that its removal should be based on needs more than anything else.
Amid a growing trend to have wisdom teeth extracted as early as age 18, some doctors say a third molar is useful in case the first two fail. Also, in many cases the wisdom tooth is impacted, which means it stays inside the bone without causing any discomfort.
Dr. Ritesh Kalaskar, professor and head of the department of pediatric and preventive dentistry, Government Dental College, Nagpur, said if the wisdom tooth grows in the oral cavity without any problem, then it is really helpful. “This (third molar) helps with chewing. Earlier, everyone needed it because of what we ate. We evolved and started eating soft foods, so the third molar was not needed,” Dr Kalaskar said.
Also, the size of our jaw decreased during the process of evolution, which limited the growth of wisdom teeth. “In many cases the tooth grows in a crooked way, making it difficult to clean. As a result, tooth decay occurs,” he said.
Despite the potential outcome, Dr Kalaskar says it is not advisable to remove it at the follicular (primary) stage. “If the first permanent molar is removed, the crown of the wisdom tooth replaces it. So how can we say that a sage is not required?’ he said.
A government dental school also said that the wisdom tooth is wrongly considered as a residual part. “The appendix is also a residual organ, but does everyone remove it? “The wisdom tooth, if it’s not a problem, should be left as it is,” he said. The impacted wisdom tooth can be straightened as it “has strategic value”.
Dr Rahul Atara, a dental surgeon, said that if the extraction is done at the follicular stage, then the trauma and post-operative complications are less. “We need to check the angle of the follicle (tooth development). It may happen that it comes out straight,” he said.
Dr Vipin Dehane, another dentist, said wisdom tooth extraction before sunrise is the best option. “Extracting the follicle stage requires less bone cutting, there are fewer future complications. In case of impacted eruption, brushing is difficult, the risk of tooth decay (cavity) is high, infection and swelling of the gums, crowding of other teeth, pain and gum disease occur,” he said.
